Claims checked

  • Lengthens telomeres and thereby lifespan

    Not studied

Longevity · healthy · Animal study

Telomere lengthening only in cell culture [2][4] and inconsistent in blood cells outside the body [3]. In mice, no longer mean lifespan, only measures in the longest-lived animals [7]; in flies 11–16% longer [6]. No study of telomeres in the body or of lifespan with epitalon in humans [1].[2][4]

  • Normalizes melatonin and the sleep-wake rhythm

    Single unconfirmed finding

Sleep · Patients: people with reduced melatonin production (older people, night shift) · Human, randomised

In a small randomized, placebo-controlled study, the urinary melatonin breakdown product rose; sleep was not measured; blinding not described [11]. Another small study from the same network [10]; no effect in isolated rat pineal glands [9].

  • Protects against cancer

    Not studied

Longevity · healthy · Animal study

Fewer breast tumors in HER-2/neu mice [8] and fewer leukemias at the same overall tumor rate in a mouse study [7]; no data in humans. In the opposite direction, epitalon lengthened telomeres in breast cancer cells [4].

Mechanism of action

Evidence map: Epitalon

Each line stands for a published finding; line style and color show where it comes from. The filter hides weaker evidence levels. Sources: see references.

Epitalon was derived from the amino acid composition of epithalamin, a polypeptide extract from the pineal gland of cattle; the two are chemically different, so their data are not interchangeable. Epitalon was developed and mostly studied by Khavinson and colleagues at the St. Petersburg Institute of Bioregulation and Gerontology; according to a 2026 review, the literature is concentrated in a few research groups [1]. The telomere claim goes back to a short communication from this group: in telomerase-negative human fetal fibroblasts, epitalon induced expression of the catalytic subunit of telomerase, telomerase activity, and telomere elongation [2]. In stimulated blood lymphocytes from 11 donors, telomeres lengthened in 5, shortened in 2, and remained unchanged in 4 [3]. An independent group from London found in 2025 longer telomeres in normal epithelial cells and fibroblasts via more hTERT and telomerase, but in breast cancer cell lines via the telomerase-independent ALT mechanism [4]; three figures in the paper were later corrected as erroneous [5].

The lifespan claim rests on animal experiments from the same group. In fruit flies that received epitalon only at the larval stage, the adult animals lived 11–16% longer [6]. In mice (54 per group), lifelong administration did not change mean lifespan; the longest-lived 10% lived 13.3% longer, maximum lifespan rose by 12.3%, leukemias were less frequent, and the total number of tumors was the same [7]. In mice overexpressing HER-2/neu, fewer and smaller breast tumors developed [8]. The presumed route via the pineal gland is contradictory: in isolated pineal glands of young and old rats, epitalon did not change melatonin release [9].

In humans there are only a few small studies, all from the network of the developer group. In older people with reduced melatonin production, nocturnal melatonin rose after epitalon [10]. In a small randomized study in women on night shifts, urinary excretion of the melatonin breakdown product 6-sulfatoxymelatonin rose 1.7-fold, and clock genes in blood cells moved closer to normal values; sleep was not measured [11]. An older study in retinitis pigmentosa reported a positive effect in 90% of cases, without described randomization or blinding [12]. The frequently cited 1.6- to 1.8-fold reduction in mortality in older people concerns the extract epithalamin [13]. Epitalon has been detected in illegal pharmaceutical preparations [14]; pharmacokinetics and safety in humans have not been studied according to FDA documents [1].

Assigned to the site of action Nutrient switches AMPK and mTORC1 at the lysosome, autophagy and senescent cells; not drawn in its figure: target telomerase (hTERT), not drawn in this figure.

Routes of administration and bioavailability

How much of a compound reaches the blood or target tissue depends on the route into the body and on the dosage form. All routes of administration explained

  • Subcutaneous (under the skin) · Solution

Used in the lifespan and tumor studies in mice [7][8]; no pharmacokinetic data [1].[7][8]

    Animal study

  • Sublingual (under the tongue)

Used in the night shift study [11]; uptake into the blood not measured.

    Human, randomised

  • Intramuscular · Solution

Used in the melatonin study in older people [10]; no pharmacokinetic data [1].

    Human, non-randomised

  • Oral (swallowed)

In old rats, oral administration changed nutrient uptake in the small intestine; this does not demonstrate uptake of the intact peptide into the blood [1].

    Animal study

  • Intranasal (nose)

In stressed rats, administration into the nose changed activity markers of the pineal gland; small study, not repeated [1].

    Animal study

Human studies

  • Nocturnal melatonin in older people

controlled with placebo and epithalamin groups; randomization and blinding not clearly described · Patients: older people with reduced melatonin production · n = 40 · 10 days of treatment

    Measured: nocturnal plasma melatonin

Result: Increase in nocturnal melatonin, especially at low baseline levels. Group sizes (15 epitalon, 15 epithalamin, 10 placebo) according to the review [1]; original paper in Russian.[10]

    Human, non-randomised

  • Melatonin and clock genes in night shift work

randomized, placebo-controlled; blinding not described · Patients: middle-aged women on night shifts with reduced melatonin production · n = 40 · 20 days

Measured: urinary 6-sulfatoxymelatonin, expression of Clock, Csnk1e, Cry2

Result: 6-sulfatoxymelatonin 1.7-fold higher; clock genes closer to normal values. No sleep endpoint, adverse events not systematically recorded [1].[11]

    Human, randomised

  • Retinitis pigmentosa

controlled against standard therapy; randomization and blinding not described · Patients: inherited retinal degeneration · n = 162 · 10 days of treatment

Measured: retinal function, visual acuity, visual field

Result: Positive effect reported in 90% of cases; total number according to the review [1], not independently replicated.[12]

    Human, non-randomised

Risks and warnings

  • Missing safety data

The FDA found no clinical safety data to assess whether epitalon harms humans; toxicity, immunogenicity, genotoxicity, and long-term cancer risk have not been studied. The animal studies were not designed as toxicity studies [1].

    No data

  • Theoretical: telomere lengthening in cancer cells

In breast cancer cell lines, epitalon lengthened telomeres via the ALT mechanism [4][5]. Whether this can promote tumor growth in the body has not been studied; the review names the cancer risk as unresolved [1]. The antitumor findings in mice [8] do not replace testing for cancer risk.

    Laboratory

  • Unclear identity and purity

Epitalon has been detected in illegal pharmaceutical preparations [14]; product quality is an open question according to the review [1].

    No data

Fact check: common claims

  • partly supported

    “Epitalon lengthens telomeres”

Shown in cell culture, also by an independent group [2][4]; inconsistent in blood cells [3]. Measurements in the bodies of humans are lacking [1].

  • not supported

    “Epitalon extends lifespan”

In mice no longer mean lifespan, only longer life of the longest-lived animals [7]; data from one research group. Not studied in humans [1].

  • not supported

“Studies show almost halved mortality in older people”

These figures come from the bovine extract epithalamin, partly combined with thymalin, not from synthetic epitalon [13]; the studies come from the same research network [1].

  • not supported

    “Epitalon protects against cancer”

Only tumor models in mice [7][8]; no data in humans. In cancer cell lines it lengthened telomeres [4].

Open questions

  • Can telomere and lifespan findings be replicated in independent, blinded animal studies?
  • Does epitalon reach the blood intact after injection, ingestion, or sublingual administration, and how long does it stay there?
  • Does epitalon change telomeres in the human body, and does it also promote cancer cells in the process?
  • Does epitalon improve sleep in a sufficiently large, blinded study?
